2007. Employees of Eastbay Acne & Skin Care Clinic, Inc., sales
representatives and distributors, advertising and promotion agencies and
members of their immediate families are not eligible. Void where

To Participate:
Visit
www.skinclinic.com and place a merchandise order online, or visit
the clinic in Concord, CA - one sweepstakes entry for each $1
purchased*. Online and In-Store purchases qualify for a sweepstakes
entry. Products must be paid for and delivered before the drawing date
to be eligible for the sweepstakes. Any merchandise returned for refund will
disqualify the entry for that purchase. Alternately, sign the
EBAC Guest Book for a complimentary entry
into the Sweepstakes. The name of the purchaser or Guest Book entry
will be the name entered in the sweepstakes - no substitutions are allowed.
All eligible customers will be automatically placed in the drawing.
There are no restrictions on the number of times you can enter the
Sweepstakes, although Guest Book entries are limited to one per person.
False or deceptive entries will disqualify the entrant, and duplicate Guest Book
entries will be removed. Orders placed before April 1, 2008 qualify for the
sweepstakes.

Prizes/Approximate Retail Value (ARV)
One (1) Grand Prize: Trip for two (2) persons to
Maui, Hawaii (ARV: $2,500). Grand Prize includes round trip coach air fare
for two (2) persons from a major commercial US airport nearest the winner's
residence, standard double occupancy condominium accommodations at Kahana Reef
Condominium Resort for seven (7) nights, beginning on the Grand Prize Winner's
choice of a Saturday or Sunday between May 1, 2008 and
December 15, 2008. Winner and guest must travel
together on the dates selected. Travel and lodging accommodations subject to
availability. Prize winner is responsible for any other expenses incurred as
a result of the trip, including, but not limited to meals, beverages, ground
transportation, airport charges, taxes, and all other incidental charges.
One (1) First Prize: $200.00 Shopping spree at Eastbay Acne & Skin Care
Clinic, or online at Eastbay Skin Care; Ten (10) Second Prizes: $50.00 gift
certificate, good for the purchase of products at Eastbay Acne & Skin Care
Clinic, excluding services; Odds of winning are dependent upon the number of
valid entries received.
Drawing/Conditions of Participation:
Winners will be selected in a random drawing
conducted by an independent judging organization on or about April 14,
2008. Grand Prize winner will be notified by email, and then by certified
mail. Grand prize winner will be required to submit an affidavit of
Eligibility and Liability/Publicity Release which must be signed, notarized
and returned within a specified time, or prize will be forfeited. Companion
of Grand Prize winner will be required to submit an affidavit of Eligibility
and Liability/Publicity Release which must be signed, notarized and returned
within a specified time prior to the issuance of travel documents. By
participating, winners agree to release Eastbay Acne & Skin Care Clinic,
Inc. from all liability with respect to this sweepstakes, and agree to allow
the use of their names, pictures, and voices for advertising, publicity, and
promotional purposes without any additional compensation. Eastbay Acne &
Skin Care Clinic, Inc. and promotional partners will have no liability
whatsoever for any personal injuries, losses or damages of any kind
resulting in whole or in part from possession, use or misuse of any prize.
Any dispute will be resolved through arbitration. Any and all applicable
taxes on a prize are the sole responsibility of the prize winner. The
Sweepstakes Sponsor is not responsible for typographical or other errors in
printing, offering or administration of sweepstakes or announcement of
prizes.

self-addressed, stamped envelope (SASE) to
